Outreach & Case Management
Through centre-based and outreach case management, Youth Rezolutions undertakes a client centred, strengths based approach aimed at empowering and working in partnership with clients to effectively meet their individual needs and become self-sufficient.
The Youth Rezolutions caseworker will endeavour to meet the client's basic and immediate needs, as well as assess ongoing needs and develop a case plan to agree on a way forward.
Self-referrals are welcome as well as referrals from agencies, family or friends working with the young person. We operate on the 'no wrong door' principle that all person's presenting to our youth service need to be engaged, regardless of whether or not they are within our eligibility criteria. We will take responsibility for either providing a service or working with the individual to access and experience a smooth transition to an alternative more suitable agency.
A referral form should be completed in the first instance. Requests are generally assessed within 48 hours. Alternatively please contact the office during business hours.
